Indications Liptor EZ is indicated as an adjunct to diet and lifestyle modification for patients with elevated cholesterol levels who are at increased risk of atherosclerotic vascular disease. It is used for:

Primary hyperlipidemia: Reduction of total‑C, LDL‑C, Apo B, triglycerides, and non‑HDL‑C, and increase of HDL‑C in patients with heterozygous familial or non‑familial hyperlipidemia, or mixed hyperlipidemia.
Homozygous familial hypercholesterolemia (HoFH): Reduction of total‑C and LDL‑C as an adjunct to other lipid‑lowering treatments (e.g., LDL apheresis) or when such treatments are unavailable.

Pharmacology Atorvastatin: Inhibits HMG‑CoA reductase, reducing hepatic cholesterol synthesis and increasing LDL receptor expression, thereby lowering plasma LDL and total cholesterol. Ezetimibe: Inhibits intestinal cholesterol absorption by targeting the sterol transporter NPC1L1, reducing hepatic cholesterol stores and enhancing clearance from blood. The mechanism complements that of statins.

Dosage & Administration

Usual range: 10/10 mg – 80/20 mg once daily, with or without food.
Starting dose: 10/10 mg or 20/10 mg daily; for > 55 % LDL‑C reduction, start with 40/10 mg daily.
HoFH: 40/10 mg or 80/10 mg daily as adjunct therapy.
Hepatic impairment: Contraindicated in active liver disease or persistent transaminase elevation.
Renal impairment: No dosage adjustment required, but monitor for myopathy.

Interactions Risk of myopathy increases with fibrates, niacin, cyclosporine, or strong CYP3A4 inhibitors (e.g., clarithromycin, HIV protease inhibitors, itraconazole). Avoid co‑administration with cyclosporine or gemfibrozil. Monitor INR if used with warfarin. Caution with digoxin, oral contraceptives, and colchicine.

Contraindications Active liver disease, unexplained persistent transaminase elevation, or hypersensitivity to any component.

Side Effects Common: myalgia, rhabdomyolysis, liver enzyme abnormalities, abdominal pain, elevated hepatic enzymes.

Pregnancy & Lactation Pregnancy Category X. Contraindicated in pregnancy and lactation. Cholesterol is essential for fetal development; lipid‑lowering therapy offers no benefit during pregnancy.

Precautions & Warnings Monitor for myopathy and rhabdomyolysis, especially with colchicine. Obtain liver enzyme tests before starting therapy and as clinically indicated. Discontinue if serious liver injury or jaundice occurs.
